POSITION

Coordinates and oversees the medical care and treatment of a caseload of individuals with intellectual and developmental disabilities. Works collaboratively with physicians, interdisciplinary teams, individuals in services, providers, and families to promote positive outcomes. Applies clinical knowledge to assess individuals and provide consultation and training. Assesses plans, implements, coordinates, monitors, and evaluates services required to support the individual’s health needs.

RESPONSIBILITIES

  • Provides delegation and consultation to unlicensed providers, families, and Support Inc staff.
  • Provides medical consultation to interdisciplinary team members
  • Teaches and trains providers, families, and staff in:
    • Medication Administration (QMAP) and MRB training
    • Individual specific medical documentation
    • Evaluate signs of illness or dysfunction that warrant medical or nursing intervention
    • Basic skills required to meet the health needs of the individual.
    • Utilizing specialized medical equipment
  • Applies knowledge of nursing practices, state and federal regulations, guidelines, and procedures to ensure compliance with home and community-based services.
  • Completes and documents evaluations and assessments to identify training and support needs to ensure continuity of care.
  • Develops care plans, reviews medical documentation, and health tracking.
  • Develop effective working relations and cooperate across teams throughout the entire case management process.
  • Help individuals receiving services and their caretakers make informed decisions by acting as their advocate regarding their clinical status and treatment options.

MINIMUM QUALIFICATIONS & LICENSE/CERTIFICATION REQUIREMENTS

  • RN with current Colorado license.
  • Two years professional healthcare experience

PREFERRED QUALFICATIONS

  • Experience as a Registered Nurse working with individuals with intellectual and developmental disabilities.
  • Knowledge and experience of physical conditions and treatment protocols for conditions of the IDD population.
  • Nurse case management experience.

WORK ENVIRONMENT

The work environment is primarily a work from home position. This role requires occasional travel to our offices (Aurora, Colorado Springs, Pueblo) and the homes of the individual’s we serve (across the front range). This position requires a personal vehicle for transportation in order to effectively monitor and coordinate care.

SCHEDULE

Monday- Thursday, 10 hours per day. This position offers a flexible work schedule that may vary based on need. This position is not required to work weekends or carry an on-call phone.

LANGUAGE SKILLS

Ability to read, analyze and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, consumers, guardians, family members, and the general public.

REASONING ABILITY

Ability to solve practical and complex problems and critically reason through a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. Ability to prioritize work needs to effectively manage time.

PHYSICAL DEMANDS

While performing the duties of this job, the employee is regularly required to reach with hands and arms and talk or hear. The employee frequently is required to drive. The employee must occasionally lift and/or move up to 50 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, depth perception, and ability to adjust focus.
